Recently, there’s one health trend that’s been dominating conversations in my office. Whether it’s the sight of collagen powders on store shelves, supplements at the pharmacy, or the endless stream of social media ads, it seems collagen is absolutely everywhere.

Many of my patients ask me with a mix of hope and skepticism: “Dr. Shahnoos, does this stuff actually work? Is it hype, or is it health?”

In this post, I’d like us to explore the science behind collagen, just as we would during a personal consultation, so we can move beyond marketing buzz and focus on what medical research truly reveals. My goal is to help you make well-informed decisions to best support your health.

Why is Collagen so Important?

First, let’s establish what we’re talking about. Collagen isn’t just a “beauty ingredient.” It’s the most abundant protein in your body. Think of it as the glue that holds everything together, or perhaps, more accurately, the steel framework of a skyscraper.

Collagen provides structure, strength, and support throughout your entire system. It’s in your bones, your muscles, your tendons, and yes, it makes up about 75% to 80% of your skin. It gives your skin that youthful glow, firmness, and resilience we all cherish.

But here’s the biological reality we must face: our bodies aren’t static. As we age, our natural collagen factories begin to slow down. We lose about 1% of our collagen per year starting as early as our mid-20s. This isn’t a failure of your body; it’s a natural progression. And that’s why we start to see fine lines, thinner skin, and a few more creaks in our joints than we used to.

The Great Debate: Hype vs. Evidence

I believe in transparency, so I’ll be honest with you: the debate around collagen supplements is ongoing in the medical community. According to the Cleveland Clinic notes, the benefits of collagen are sometimes more hype in the media than the evidence currently supports. We need more large-scale published research to be absolutely certain of health benefits.

That said, “more research needed” doesn’t mean “no evidence exists.” When we look at the current medical literature, especially regarding hydrolyzed collagen peptides, the data is actually quite promising.

What the Science Actually Says

As an internist, I look for randomized controlled trials, which are the gold standard of medical research.

Recent analyses show that oral collagen supplementation can offer measurable benefits. We aren’t guessing here: we’re seeing statistically significant improvements. Here’s what the latest science tells us:

It’s All About the “Peptides”

You can’t just eat a hunk of collagen (think a gristly steak) and expect it to travel straight to your face. Digestion breaks proteins down. But hydrolyzed collagen is different. It’s broken down into tiny chains of amino acids called peptides. Research suggests specific bioactive peptides (some with names like Gly-Pro-Hyp and Pro-Hyp) are absorbed into the bloodstream. They then accumulate in target tissues like your skin and joints. Once there, they act like little messengers, nudging your body’s extracellular matrix to wake up and produce new Type I collagen.

Skin Hydration and Elasticity

A meta-analysis of 26 different trials involving more than 1,700 patients showed that hydrolyzed collagen significantly improved skin hydration and elasticity. Hydrated, elastic skin looks plump, radiant, and healthy, and become skin that resists the deepest wrinkles.

Joint Health Benefits

We can’t talk about wellness without talking about mobility. Double-blind trials have shown significant reductions in osteoarthritis symptoms and joint pain with collagen peptides. For many patients, collagen isn’t just about looking good: it’s about moving with less pain.

Drinking Just “Bone Broth” is Not Enough

A lot of people ask, “Dr. Shahnoos, can’t I just drink bone broth?”

I love a warm cup of bone broth as much as anyone. It’s soothing and nutritious. But from a therapeutic medical perspective, the results are not consistent.

When we analyze bone broth in the lab, the concentrations of key amino acids such as glycine, proline, hydroxyproline are inconsistent and typically far lower than what you get in a standardized supplement. You simply can’t guarantee you’re getting a therapeutic dose from broth.

Most clinical studies that show real results used daily doses of 2.5 to 10 grams of hydrolyzed collagen for 8 to 24 weeks. To hit that level of consistency and potency, a high-quality standardized supplement is generally more reliable than relying on broth alone.

My Holistic Approach to Collagen

So, do I recommend collagen? The answer is a nuanced “Yes” as long as it is part of a bigger picture. Taking a supplement while neglecting your overall health is like painting the walls of a house while the foundation crumbles.

Stop the Damage

  • Sun protection: UV radiation is the number one enemy of collagen. Wear sunscreen every single day.
  • No smoking: Smoking reduces blood flow and directly damages collagen and elastin. If you smoke, let us help you quit. It’s the single best thing you can do for your skin and your internal organs.
