Your Guide to Understanding BOTOX® and Dermal Fillers

Entering the world of cosmetic injectables can feel overwhelming, especially with all the information (and misinformation) available online. When patients visit me at Novel U Wellness and Med Spa in Long Beach, CA, two of the most common questions I hear is, “What’s the difference between BOTOX® and dermal fillers?” and “Which one is the better choice?” These treatments serve very different purposes and understanding that distinction is the first step toward achieving natural, balanced results: BOTOX® relaxes targeted facial muscles to soften expression-related lines, while dermal fillers restore volume that has been lost over time.
During your consultation at Novel U Wellness, we’ll evaluate your specific concerns and develop a personalized treatment plan. Some patients need only BOTOX®, others benefit most from fillers, and many achieve their best results with a thoughtful combination of both. I always tell my patients: if someone can tell you’ve had “work done,” we’ve done too much. My goal is to enhance your natural beauty, not transform you into someone else. This is why we often take a conservative approach, especially if you’re new to injectables. We can always add more, but it’s much harder to reverse overfilling.
In the Q and A section below, you’ll learn what you need to know about neuromodulators and dermal fillers: how they work, what they cost, what to expect during treatment, and most importantly, how to choose a qualified provider. My goal is to give you the knowledge you need to make informed decisions about your care.
BOTOX® and Dermal Fillers: What’s the Difference?
1. What is the fundamental difference between BOTOX® and dermal fillers?
In simple terms, BOTOX® relaxes movement, while fillers replace lost structure.
Think of BOTOX® and other neuromodulators (like Dysport) as muscle relaxers. They work by temporarily blocking nerve signals to specific facial muscles, preventing them from contracting. This is perfect for treating “dynamic wrinkles” – lines that appear when you move your face, like forehead furrows, crow’s feet around the eyes, and the “11 lines” between your brows. When those muscles relax, the overlying skin smooths out.
Dermal fillers, on the other hand, are volumizers. Most fillers used at Novel U Wellness contain hyaluronic acid, a naturally occurring substance in your body that attracts and holds moisture. Fillers physically restore lost volume and fill in “static wrinkles” – lines that are visible even when your face is at rest, and are excellent for addressing hollowing in the cheeks, smoothing nasolabial folds (the lines from your nose to the corners of your mouth), enhancing lips, and defining the jawline.
2. How much do BOTOX® and dermal fillers typically cost, and what factors affect pricing?
BOTOX® is priced by the unit, with costs typically ranging from $10 to $20 per unit in 2026. You only pay for what you use. A standard treatment for the forehead and crow’s feet usually requires 30 to 40 units, putting the total cost between $300 and $800 per session. The amount needed varies based on your muscle strength, the area being treated, and your aesthetic goals.
Dermal fillers are priced by the syringe, with costs generally ranging from $600 to $1,200 per syringe. Most patients pay for the entire vial, even if we don’t use every last drop (though I always find ways to optimize your results with what we have). The number of syringes you need depends on the area being treated and the degree of volume loss.
When considering the financial commitment, you’ll want to factor in maintenance. BOTOX® typically lasts three to six months, meaning you’ll need three to four treatments per year. Fillers generally last nine to 18 months, depending on the product and placement, so while they’re a larger upfront investment, you won’t need them as frequently. At Novel U Wellness, we’ll create a personalized treatment plan that fits your budget and goals.
3. What does the injection process feel like? How much discomfort should I expect?
Pain tolerance varies from person to person, but most patients are pleasantly surprised by how manageable the discomfort is during injectable treatments. For patients who are particularly sensitive, we can apply a topical numbing cream before your treatment at Novel U Wellness. I want you to feel as comfortable as possible throughout your experience.
BOTOX® injections feel like small pinches or mosquito bites. The needles are very fine, and the solution is injected superficially into the muscle. Most patients describe it as a brief sharp sensation that’s over in seconds. The entire treatment usually takes about 10 to 15 minutes.
Dermal filler injections can involve a bit more sensation because the product is often injected deeper into the tissue. You might feel pressure or a tugging sensation, especially when treating areas like the cheeks or jawline. However, most modern fillers contain lidocaine, a numbing agent mixed right into the gel, which provides comfort during and after the procedure. Some patients notice a slight crunching sound when fillers are placed near bone, particularly in the nose or chin area. This is completely normal and just the sound of the filler being deposited precisely where it needs to go.
4. What kind of results can I realistically expect, and how soon will I see them?
Both BOTOX® and fillers deliver beautiful results, but they work on different timelines.
With BOTOX®, you won’t see immediate results. The neuromodulator needs time to block the nerve signals to your muscles. Most patients start noticing a difference within three to five days, with full results appearing around two weeks after treatment. The effect is a softer, smoother appearance in the treated areas. Your expressions will still be natural: you’ll just have fewer lines when you smile, frown, or raise your eyebrows.
Dermal fillers provide more immediate gratification. You’ll see volume restored and lines smoothed right after your appointment, though some initial swelling may enhance the effect temporarily. Over the next few days, as any swelling subsides and the filler settles into place, you’ll look like you’ve been getting good sleep and drinking plenty of water.
5. How should I prepare for my injectable appointment?
Proper preparation can make a significant difference in your results and your recovery. I recommend following what I call the “seven-day rule” to minimize bruising and swelling.
One week before your appointment, avoid blood-thinning medications and supplements. This includes aspirin, ibuprofen (Advil, Motrin), naproxen (Aleve), fish oil, omega-3 supplements, vitamin E, ginkgo biloba, and garlic supplements. If you take any prescription blood thinners for a medical condition, do not stop them without consulting your primary care physician. Please let me know about all your medications during your consultation.
Skip alcohol for 24 to 48 hours before your treatment. Alcohol dilates blood vessels, which increases your risk of bruising. Stay well-hydrated in the days leading up to your appointment, especially if you’re getting dermal fillers. Since hyaluronic acid attracts water, good hydration helps your fillers look their best.
Schedule your appointment strategically. If you have an important event coming up, plan your treatment at least two weeks in advance. This gives any bruising or swelling time to completely resolve.
6. What should I do (and avoid) after my injectable treatment?
Following proper aftercare instructions is essential for optimal results and minimizing side effects.
For BOTOX® specifically, stay upright for at least four hours after your treatment. Lying down too soon could cause the product to migrate to unintended areas. Avoid touching, rubbing, or massaging the treated areas for the rest of the day. Skip your workout for 24 hours. Increased blood flow from exercise can potentially cause the neuromodulator to move before it has fully settled into the target muscles.
For dermal fillers, avoid strenuous exercise, saunas, steam rooms, and hot yoga for 24 to 48 hours. Heat can increase swelling. Sleep with your head elevated on an extra pillow for the first night or two to minimize facial puffiness. Don’t massage the treated areas unless I specifically instruct you to do so. While some areas benefit from gentle massage, others should be left completely alone to allow the filler to set properly.
For both treatments, apply ice or cold compresses to reduce swelling and bruising. Use Arnica gel or tablets to help minimize bruising. Avoid dental work, facials, or other facial treatments for at least one week after your injections.
If you develop any concerning symptoms, such as severe pain, skin blanching (turning white), vision changes, or difficulty speaking, contact me or your provider immediately. These symptoms are extremely uncommon but require prompt attention.
7. How do I choose a qualified provider for injectable treatments?
Always verify that your provider is board-certified. Board certification means the practitioner has completed rigorous specialty training in their field and continues to meet ongoing educational requirements. For cosmetic injectables, look for board-certified dermatologists, plastic surgeons, or physicians like me who are board-certified in internal medicine with specialized training in aesthetic procedures.
Be cautious of providers who learned cosmetic procedures through brief weekend courses or online certifications. Injecting BOTOX® and fillers safely requires in-depth knowledge of facial anatomy, understanding of potential complications, and the medical training to address them if they occur. A qualified provider should be able to answer detailed questions about your medical history, current medications, and how these factors might affect your treatment.
Ask to see before-and-after photos of actual patients. Look for results on people with similar features, skin types, and concerns as yours. The photos should show natural-looking enhancements, not overdone or artificial results.
Trust your instincts. If a provider seems rushed, doesn’t listen to your concerns, or pressures you to buy more treatment than you’re comfortable with, walk away. 8. What are the potential risks and side effects I should know about?
While BOTOX® and dermal fillers are generally very safe when administered by a qualified provider, all medical procedures carry some risk. Common side effects are typically minor and temporary. These include bruising, swelling, redness, and tenderness at injection sites. You might develop small bumps under the skin that usually resolve within a few days. Some patients experience mild headaches after BOTOX®, which can be managed with acetaminophen (Tylenol).
Less common but more significant complications include asymmetry (uneven results), which can usually be corrected with additional product or time for the treatment to settle. Drooping of the eyelid or eyebrow can occur with BOTOX® if the product migrates, though this is temporary and resolves as the neuromodulator wears off. With fillers, there’s a small risk of nodules or lumps forming, which may require treatment with hyaluronidase, an enzyme that dissolves hyaluronic acid fillers.
The most serious (but very uncommon) complication is vascular occlusion, where filler accidentally enters a blood vessel and blocks blood flow. This is a medical emergency that requires immediate treatment. Warning signs include sudden severe pain, skin blanching (turning white), or development of a dusky discoloration. This is why anatomical expertise is so important, because knowing where blood vessels are located reduces this risk significantly.
9. Why does it matter if my provider uses medical-grade products?
The quality and authenticity of the products being injected into your face should be non-negotiable. Unfortunately, counterfeit cosmetic products are a growing problem. In recent years, the FDA has issued warnings about counterfeit BOTOX® and dermal fillers entering the U.S. market. These fake products may contain unknown ingredients, incorrect concentrations, or dangerous contaminants. Using counterfeit injectables puts you at risk for serious complications, including infections, allergic reactions, and poor results. If a price seems too good to be true, it probably is. Dramatically discounted treatments may indicate the use of counterfeit products, diluted formulations, or inexperienced injectors. Your face deserves the highest quality products administered by qualified professionals.

Many of my patients benefit from what I call a “liquid facelift” which is a strategic use of both products to address multiple signs of aging simultaneously. Over time, we lose volume in our faces (which fillers can restore), and we develop movement-related wrinkles from repeated muscle contractions (which BOTOX® can soften). Addressing both concerns creates more comprehensive, natural-looking results than treating just one aspect of aging. We can perform both treatments during the same appointment if appropriate. However, I sometimes recommend staging treatments, starting with one type of injectable and adding the other at a follow-up visit. This allows us to assess results carefully and adjust for optimal outcomes.
