This decision represents a victory for evidence-based medicine and for the countless women who have suffered unnecessarily because of outdated, fear-based warnings.
Let me share with you what this means, why it matters, and most importantly—how it might change your life or the life of someone you love.
Understanding the Historic Context
To appreciate the magnitude of this FDA decision, we need to go back to 2002. That’s when the Women’s Health Initiative study was released, and it sent shockwaves through the medical community and terrified women across the country. The study suggested that HRT increased risks of breast cancer, heart disease, stroke, and dementia.
Almost overnight, HRT prescriptions plummeted. Women who were taking hormones stopped their treatment. Doctors became afraid to prescribe them. An entire generation of women suffered through debilitating menopausal symptoms, and missed out on significant health benefits, because of fear.
But here’s what we’ve learned in the 23 years since that study: the original findings were flawed and, more importantly, they didn’t reflect how HRT should actually be prescribed or who should receive it.
The women in that 2002 study had an average age of 63—more than a decade past the typical age of menopause. They were given specific hormone formulations that aren’t commonly used today. And the study didn’t account for the crucial factor that we now know makes all the difference: timing.
What the FDA Actually Changed
The FDA has worked with pharmaceutical companies to remove references to breast cancer, cardiovascular disease, and dementia risks from HRT labeling. This doesn’t mean these risks don’t exist in certain contexts. It means the blanket warnings were misleading and not based on the best available science.
Here’s what’s important: the FDA IS keeping the warning about endometrial cancer for estrogen-only products. This makes sense because we know that women who have a uterus and take estrogen without progesterone do face this risk. That’s why we always combine estrogen with progesterone for women who haven’t had a hysterectomy: it’s established, evidence-based medicine.
This decision came after the FDA conducted a comprehensive review of scientific literature, convened an expert panel meeting, and opened a public comment period. The conclusion? The warnings were causing more harm than good by preventing women who could benefit from accessing treatment.
The Timing Hypothesis: Why When You Start HRT Matters
The single most important thing that’s emerged from two decades of research is this: timing is everything.
When women start HRT within 10 years of menopause beginning, or before age 60, the benefits significantly outweigh the risks for most women. This is called the “timing hypothesis,” and it’s revolutionized our understanding of hormone therapy.
Think of it this way: the body doesn’t respond the same way to hormones at age 52 versus age 65. When we introduce hormones closer to menopause, when the body is still accustomed to having them, we see protective effects. When we wait too long, those protective effects diminish or reverse.

It would be like studying the effects of wearing seatbelts only in people who put them on after an accident and then concluding seatbelts are dangerous.

Longevity: Perhaps most striking of all is that studies show that women who start HRT within 10 years of menopause have a reduction in all-cause mortality. They simply live longer.

Yes, There Are Still Risks (And That’s Okay)
I never want to minimize risks or make any medication sound like it’s perfect for everyone. No medication is risk-free, and HRT is no exception.
The removal of the black box warning doesn’t mean HRT is safe for everyone or that there are no risks. What it means is that for appropriately selected women (those within the timing window, without contraindications) the benefits significantly outweigh the risks.
Let’s talk honestly about what we know:
Breast Cancer: The relationship is complex. Large analyses show that HRT is not associated with increased breast cancer mortality. However, some formulations may slightly increase the risk of breast cancer diagnosis, while others (like estradiol plus progesterone) may not increase risk at all. This is why individualized treatment planning is so important.
Cardiovascular Risk: For women starting HRT early, cardiovascular benefits are substantial. The increased risk seen in older studies was primarily in women who started HRT many years after menopause.
Blood Clots: There is a small increased risk with oral HRT, which is why we often recommend transdermal formulations (patches or creams) that bypass the liver and may have a lower clot risk.
Endometrial Cancer: Real and preventable. For women with a uterus, we always combine estrogen with progesterone.

If you stopped HRT years ago because of the 2002 panic, it may be worth reconsidering. Many women unnecessarily discontinued treatment and have been suffering ever since.
