For chronic migraine (≥15 headache days/month), multiple randomized controlled trials and meta-analyses—including the pivotal PREEMPT 1 and 2 studies—demonstrate that onabotulinumtoxinA reduces headache days by approximately 2–3 days per month compared to placebo, with pooled data showing a reduction of 8.4 headache days versus 6.6 for placebo over 24 weeks.Improvements in migraine-related disability and health-related quality of life are also consistently reported. Real-world data confirm these benefits and suggest durability over at least one year.[9] The American Academy of Pain Medicine guideline supports its use via the PREEMPT protocol (155–195 units injected every 12 weeks).

Safety: OnabotulinumtoxinA is generally well tolerated, with most adverse events being mild or moderate (e.g., neck pain, muscle weakness, injection-site pain) and declining with repeated cycles.No serious systemic adverse events have been reported.

For episodic migraine ,However, some recent prospective studies suggest possible efficacy, but these findings are not yet reflected in guideline recommendations or regulatory approvals.
