It can affect your whole body, including your skin. You might notice this during stressful seasons. For example, your skin may feel drier, more reactive, or slower to recover. Therefore, physicians often look at your skin health through a whole-body lens.

The stress-skin axis is the connection between emotional stress, hormones, and visible skin changes. In other words, your brain and skin are always communicating.

Furthermore, chronic stress hormones can affect the following areas:

  • The protective skin barrier
  • Collagen production and structure
  • Hydration levels and retention
  • Pigment production
  • Wound healing and repair

Over time, chronic low-grade inflammation may also contribute to visible skin aging. However, this does not mean stress “ruins” your skin. Every patient is unique, and stress is only one factor among many.

What is the HPA axis, also known as the stress-skin axis?

The stress-skin axis describes how stress affects the skin. First, the brain senses stress and turns on the body’s stress response. One major part of this response is the hypothalamic-pituitary-adrenal axis, or HPA axis.

Simply put, the HPA axis works like a hormone alarm system. For example, it helps the body release cortisol, which is one of its main stress hormones.

At the same time, the skin does not just receive signals from the brain. In addition, it has its own local stress response system. Therefore, skin cells can respond to stress directly. Over time, this local response may affect how the skin looks, feels, and repairs itself.

How does stress reach the skin?

Stress can reach the skin in more than one way. First, stress turns on the HPA axis system. As a result, the body releases stress hormones and chemical messengers.

These key messenger signals include:

  • Cortisol: One of the body’s primary stress hormones.
  • CRH (Corticotropin-Releasing Hormone): The signal that starts the stress response in the brain.
  • ACTH (Adrenocorticotropic Hormone): The signal that tells the adrenal glands to release cortisol.

At the same time, stress turns on the SAM system (sympathetic-adrenal-medullary system). Consequently, this system releases “fight or flight” chemicals like epinephrine and norepinephrine. In short, stress affects the skin through both hormone signals and nerve pathways.

Can the skin make its own stress hormones?

Yes, the skin can produce its own stress hormones. Our skin has a local HPA axis. Therefore, skin cells can produce stress-related signals independently.

Two main cells participate in this local process:

  • Keratinocytes: The primary protective cells in the outer skin layer.
  • Fibroblasts: Deeper cells that produce collagen and support skin structure.

Because these cells release stress mediators, the skin is both a target and a source of stress signals. In other words, stress does not only come from the brain. The skin actively participates in the stress response, too.

What does cortisol do to the skin?

Cortisol directly affects the skin barrier. This barrier is the protective layer that keeps moisture in and keeps irritants out. When stress persists, the outer skin layer may produce more active cortisol locally.

As a result, the skin experiences higher stress activity right where it repairs itself. Additionally, cortisol reduces the production of essential fats needed to keep the skin barrier strong. Consequently, the skin becomes drier, less hydrated, and more vulnerable.

How can stress affect the skin barrier?

Chronic stress can weaken the skin barrier over time. When cortisol levels stay high, the outer layer of the skin holds less water. Put simply, the skin becomes less capable of maintaining hydration.

At the same time, stress increases transepidermal water loss, which allows water to escape through the skin. Therefore, your skin may feel dry, tight, and irritated. Ultimately, a weaker barrier makes the skin more sensitive to allergens, irritants, and environmental stress.

 

What is inflammaging?

Inflammaging refers to long-term, low-grade inflammation that contributes to visible skin aging. This inflammation is not always dramatic or obvious. Instead, it is quiet, subtle, and persistent.

When stress becomes chronic, it triggers inflammatory messages in the skin. These messages travel through messenger proteins called cytokines.

Common inflammatory cytokines include:

  • IL-6
  • TNF-alpha
  • IL-1 beta

However, when these inflammatory signals stay active for too long, they accelerate visible skin aging. Over time, chronic stress makes it much harder for the skin to repair itself properly.

How does inflammation affect collagen?

Collagen is the main support protein that helps your skin look firm, smooth, and resilient. However, chronic stress and inflammation can damage collagen in two distinct ways.

First, stress hormones slow down fibroblasts, which are the cells that create new collagen. Second, stress increases specialized enzymes called MMPs (matrix metalloproteinases). These enzymes actively break down existing collagen and elastic fibers.

Taken together, chronic stress slows down collagen production while speeding up its destruction. Consequently, this combined process contributes to thinner, more fragile, and more wrinkled skin over time.

What are senescent cells and SASP?

Senescent cells are cells that stop functioning normally after experiencing stress or damage. Put another way, they are often called “aged” cells because they no longer behave like healthy cells.

However, senescent cells do not simply disappear from the body. Instead, they release inflammatory signals into surrounding tissue. This harmful pattern is called SASP (senescence-associated secretory phenotype).

SASP creates an inflammatory “signal cloud” containing several compounds:

  • Cytokines: Proteins that keep inflammation active.
  • Chemokines: Signals that call immune cells into the area.
  • MMPs: Enzymes that break down collagen and skin structure.

As a result, senescent cells continuously fuel the inflammatory cycle in the skin.

What are mast cells?

Mast cells are immune cells that help control inflammation. When they are activated, they release several active compounds into the skin.

These compounds cause specific skin reactions:

  • Histamine: Causes the skin to feel itchy, red, or reactive.
  • Tryptase: Amplifies inflammation and activates collagen-destroying enzymes.
  • Cytokines: Direct immune cells to stay active and release more inflammatory signals.

In stressed skin, mast cells recruit additional immune cells into the area. Furthermore, they activate MMP enzymes that degrade collagen. Consequently, inflammation begins to feed on itself, creating a continuous loop. Over time, this cycle weakens skin structure and accelerates visible aging.

 

Can stress make skin thinner or slower to heal?

Yes, chronic stress can cause skin thinning and slower wound healing. Too much cortisol thins both the outer layer (epidermis) and the deeper support layer (dermis). When these layers thin, the skin becomes significantly more fragile.

In addition, high cortisol levels delay skin repair after irritation or injury. Consequently, stress-related skin changes often manifest as wrinkles, fragility, dryness, and prolonged recovery times.

Can stress create an uneven skin tone?

Yes, stress-induced inflammation can contribute to dark spots and uneven skin tone.

Inflammatory signals stimulate a process called melanogenesis. In other words, inflammation instructs pigment-producing cells to create excess pigment. As a result, chronic inflammation frequently contributes to hyperpigmentation and uneven skin tone.

Can chronic stress affect cellular aging markers?

Yes, chronic stress affects skin aging at a deep cellular level through three main mechanisms:

  • Epigenetic changes: Stress alters how specific genes are turned on or off.
  • Shorter telomeres: Stress shortens the protective end-caps on chromosomes, accelerating cellular aging.
  • Reduced telomerase: Stress lowers the enzyme responsible for repairing and maintaining telomeres.

Taken all together, chronic stress directly speeds up the cellular aging process in your skin.

Fortunately, several strategies can help interrupt the stress-inflammation-aging cycle.
